SMS Terms & Conditions

These SMS Terms & Conditions apply to text messages sent by or on behalf of My Choice Programs, Inc. (“My Choice Programs,” “we,” “our,” or “us”).

1. SMS Consent

By opting in to receive SMS/text messages from My Choice Programs, you agree to receive text messages from us at the mobile phone number you provide.

SMS consent is voluntary. You are not required to consent to SMS/text messages as a condition of receiving services from My Choice Programs. If you do not consent to SMS/text messages, we may contact you by phone, email, mail, or another appropriate method.

The information obtained as part of the SMS consent process, including phone numbers and SMS consent records, will not be shared with third parties or affiliates for marketing purposes.

2. Types of SMS Messages

If you consent to receive SMS/text messages from My Choice Programs, you may receive messages related to:

  • Appointment, meeting, or scheduling reminders.

  • Service-related reminders.

  • Follow-up communications.

  • Conversational text messages.

  • Coordination with participants, families, guardians, providers, applicants, staff, or referral sources.

  • Employment, provider, or program inquiries, where applicable.

  • Other non-marketing communications related to My Choice Programs.

My Choice Programs does not use SMS consent to send third-party marketing messages.

3. Message Frequency

Message frequency may vary depending on your relationship with My Choice Programs and the nature of the communication.

4. Message and Data Rates

Message and data rates may apply. Your mobile carrier’s rates and terms are your responsibility.

5. Opt-Out Instructions

You may opt out of receiving SMS/text messages at any time by replying STOP.

After you opt out, you may receive one final confirmation message confirming that you have been unsubscribed. After that, we will not send further SMS/text messages to that number unless you opt in again or another legally permitted basis applies.

7. Privacy

Mobile opt-in information, SMS consent, and phone numbers collected for SMS communication purposes will not be shared with third parties or affiliates for marketing purposes.

8. Sensitive and Confidential Information

SMS/text messaging should not be used to send Social Security numbers, financial-account information, detailed medical information, medication lists, DDS referral packets, service plans, incident reports, or urgent matters.

Confidential participant, referral, medical, DDS-related, or service-related information should be sent only through the secure method designated by My Choice Programs.

9. Emergencies

SMS/text messaging is not an emergency communication system.

For a medical emergency or immediate danger, call 911 or seek emergency assistance.

10. Changes to These Terms

We may update these SMS Terms & Conditions from time to time. The updated version will be posted on this page with an updated “Last Updated” date.
