1)  Shared Living Provider:

In Shared Living, a person, couple, or family share their home and life experiences with a person that has an intellectual disability, acquired brain injury, or autism. A shared living provider is an in-house mentor who will establish a mutually supportive relationship that offers the individual the opportunity to live semi-independently, through teaching, compassion, and respect. My Choice strives to match individuals and providers who have similar interests and personalities so that all program participants can have a positive and rewarding experience. As an in-house mentor, you will be an independent contractor and receive a tax-free bi-monthly stipend that reflects the intensity of service needed for the individual. My Choice Programs will provide ongoing support, training, and supervision. You must have a spare bedroom in your home.

 

Job Description Summary: The role of the Shared Living Provider is to provide a safe home where the individual is empowered to develop relationships within the home and the community.

As a Shared Living Provider, you will be responsible to:

• Take part in mandatory training as deemed necessary by the shared living team. Some of the required training includes First Aid & CPR, Human Rights, and Fire Safety.

• Coach and assist the individual with their health needs. This will include reviewing their well-being and safety needs, as well as scheduling, transporting, and getting documentation at all medical appointments, and advocating for assistance.

• Coach and support the individual in the administration of any medication, such as ordering and supervising medication, depending on the individual’s ability and needs.

• Coach and support the individual in the management of funds by providing coaching with banking, financial record keeping, and distribution of funds according to the individual’s ability.

• Transport the individual according to his/her needs, which could include vocational program/work, medical appointments, and family and community activities.

• Evaluate /report any crisis to the shared living team and coach the individual to advocate for their needs. Coach and assist the individual to work through their crises and to re-stabilize in their environment.

• Attend and take part in funding sources (DDS) meetings and implementation of service plans and goals for the individual, as well as yearly state licensing reviews.

• Ensure 24-hour coverage of the home.

• Provide coaching and the opportunity for community integration.

• Maintain neatness and cleanliness of the home on a weekly basis, assisting and coaching the individual to keep their personal areas clean and free of hazardous materials.

• Coach and assist the individual to food shop, and plan and cook nutritional meals depending on their ability.

 

 

2)  Transitional Living In-Home Mentor:

 

 An individual taking part in our TL program has the goal of eventually living independently in his/her own apartment or home.  The TL program could last several years, depending on the support needed and the goals of the individual.  The TL in-house mentor will establish a mutually supportive relationship that offers the individual the opportunity to live semi-independently, through teaching, compassion, and respect of choice. My Choice strives to match individuals and providers who have similar interests and personalities so that all program participants can have a positive and rewarding experience. In-house mentors receive a tax-free monthly stipend that reflects the intensity of service needed for the individual. My Choice Programs will provide ongoing support, training and supervision. You must have a spare bedroom in your home.

 

Job Description Summary: The role of the Transitional Living In-Home Mentor is the same as in Shared Living.
