Shared Living

 

In Shared Living, an individual, couple, or family share their home and life experiences with a person of varying abilities. A shared living provider is willing to establish a mutually supportive relationship that offers the individual the opportunity to live the life they choose, through teaching, compassion and respect of choice. My Choice strives to match individuals and providers who have similar interests and personalities so that all program participants are able to have a positive, happy and rewarding experience. Caregivers are offered a tax free monthly stipend that reflects the level of care needed for the individual. My Choice Programs offers ongoing support, training and supervision. You must have a spare bedroom in your home to be considered for this contract position.

 

Job Description Summary: The role of the Shared Living Provider is to provide a safe home where the individual can be empowered to develop relationships within the home and the community.

Reports to: Assigned Shared Living Placement Coordinator

Functions:

 

• Participate in mandatory trainings as deemed necessary by the shared living team. Some of the trainings are, First Aid & CPR, Human Rights and Fire Safety.

• Assist the individual with their health needs. This will include reviewing their wellbeing and safety needs, as well as scheduling, transporting and obtaining documentation at all medical appointments and advocate for any assistance.

• Support the individual in administration of any medication such as ordering and supervising medication depending on the individual’s ability and needs.

• Support the individual in the management of funds by providing assistance with banking, financial record keeping and distribution of funds according to the individual’s ability.

• Transport individual according to their needs, which could include vocational program/work, medical appointments, and family and community activities.

• Evaluate /report any crisis to the shared living team and advocate for the needs of the individual. Assist the individual to work through their crises and to re-stabilize in their environment.

• Attend and participate in funding sources (DDS) meetings and implementation of service plans and goals for the individual, as well as yearly state licensing reviews.

• Ensure 24 hour coverage of the home.

• Provide the opportunity for community intergration.

• Maintain neatness and cleanliness of the home on weekly basis, assisting and ensuring that individuals in the home keep their personal areas neat and clean and free of hazardous materials.

• Food shop for the household and plan and cook nutritional meals for the individual depending on their ability.

• Perform other duties as assigned.

Transitional Living Care

 

Under our shared living residential option, we offer our TLC program.  Individuals participating in our TLC program have the ultimate goal of eventually living independently in their own apartment or home.  Placements could last several years depending on the support needed and the goals of the individual.  The Transitional Living Care Provider is willing to establish a mutually supportive relationship that offers the individual the opportunity to live the life they choose, through teaching, compassion and respect of choice.
